Thread Plug Gages - No-go goes and go does not

Where is the supervisor that's responsible and how did this go so far ? Is anyone else responsible for checking everything out as far as tooling , speeds and feeds in the programs , gauges etc . Can't believe manufacturing engineers would have to be called in to diagnose the situation .

The supervisor bounced it over to engineering (a co-worker and I). I had only been hearing about it and then asked about it once, I thought for sure I would get stuck with it. I didn't and so far do not know what come of it.

I often see multiple ideas thrown at problem then when it is fixed nobody really knows what the root cause was. That when I usually get a job.
