Try this: Swap 5 and 8 if necessary to match motor direction with switch
(don't forget to ground the motor and switch case both)

Popping the cover off, I don't think this is an actual 2601.

And this seems to be the crux of my problems, I think someone put one cover on a different switch. Not an unusual problem with buying used, but something to be aware of. The internals appear to be the hockey stick type, as opposed to the chop sticks a 2601 should have.
 
LOL I thought about that possibility- yeah someone must have switched covers or there are two versions of 2601

Does the motor have 6 wires or 7? I only see 6 in the photo but on the motor dataplate it shows 7
The pic of the test hookup in post #9 doesn't make sense to me
