Type B Vs. Type Km Er Nuts?

Amazon is sending me out a second one. If it also turns out to be defective, then I'll post a review and buy the other one.

Has anyone ever heard of an ER nut not having the offset flange?

Yes but not from personal experience. I have read that the offset flange creates imbalance at high RPMs. The RPM rating of the Pro series nut might be an indication of why there is no eccentric flange.

There are high speed nuts with clips or slots to provide better balance at speed. Does your pro nut have any provision for extracting the collet?

I had the same thought as well. However, if that's the case, it should have been noted in the description.

It doesn't have any provision for extraction.
