The bolts Bob12345 showed look like toilet bolts.
Machining the 45* sises is not s big deal. If your mill head tilts, that is one way. A multi-tooth 90* countersink does a good job as a milling cutter, that is the second way. An end mill can be re-sharpened with a 45* angle, that is the third way. Those three approaches to the problem allow you to create a long bar of the needed profile. To do a single nut, you can clamp it in the vise with the part tilted to 45*. Use a standard end mill to make the cuts.
