Order of operation for shaft repair.

So I need to repair a short shaft for a boat lift winch. The threads were stripped on the threaded portion that the "hand wheel" is connected to and that brings up my first question. Is there a name for a threaded shaft that has two flats 180 degrees apart to act a bit like a key in that it prohibits Turing on the shaft? Second question is it better to mill the flats first then use a die to cut the threads? Or do you cut threads then mill the flats. I assume both would work but is one method preferred?

It would be very difficult to get a die to cut on a flatted shaft. It's even difficult to clean up existing threads on one. The die will want to cam off of the edges of the flats.
 
While you could cut the threads on a lathe with the flats previously cut, you will have a better quality thread with the non-interupted cut.
