If you don't turn the spindle, you cannot use the gear train to drive the lead screw to move the carriage. The 32T gear on the back end of the spindle is what drives the gear train.

If you pull one gear out of the change gear train, then you can drive the lead screw (somehow) without the spindle turning. Otherwise not.

And if you have it, I would definitely use the follow rest. Actually, this is the one case where using the follow rest is easy, as you won't have to readjust it after every pass.
 
If you are talking about thin wall al tubing. You can make a mount for a die grinder or Dremel tool for your tool post and use a small router bit to cut the groves in one pass. And hand cranking wouldn't be much of a problem.
