Millrite Needs Parts For Bed

I have a Millrite MV, I need both the "X" and the "Y" mounts for the screws. My machine was set up using air over hydraulic. The screws were removed, I have the nut on the front and back movement, it looks in good shape. Any help would be appreciated.

The A over H was taken off a long time ago. The hydraulic cylinder moved the bed and the air switched from x to y in a cycle and the clamping set up.
