How to wire 1340-GT single phase?

My 1340-GT came without a power cord. I purchased a 6-15 to ROJ (15 Amps). It has 3 colored wires: Green, Black, and Gray.

Now, I don't have any idea how to wire it. The manual just says to use the R and S terminals. It doesn't say which color wires go to which terminal, nor does it say what to do with the grounding wire.

Any help appreciated.
 
obviously you don't want a 115v motor being plugged into a 240v circuit

but i'll assume ,for arguments sake, that you'll be hooking up to 240volts- (the wiring is the same for 115v if you only have 2 motor terminals)
the color wire that goes to the R & S terminals is inconsequential - either black or gray would be fine in either order
the motor should have a green grounding screw (sometimes a black screw), that resides in the end cap near the cover plate
if you do not have a ground terminal or other hole to put a screw into, the ground wire could be hooked up anywhere on the outside of the motor provided the chosen spot is not isolated electrically from the motor frame, the green grounding wire goes here
 
Thanks. Here's a picture of the box; it looks like maybe the large silver screw is meant for the grounding wire? If so, should I be concerned that there is already one wire leading to it?

the silver screw would be fine to use as grounding screw
the symbol on the white jacket is the electrical ground symbol

no need for alarm, you can make the ground connection here
stacking grounds is electrically ok
