This is the threaded locking quill shoe. It goes on the side with the threads on the lock bolt.
This is the Unthreaded locking quill shoe. It goes on the side nearest the front of the lock bolt.
This is where the quill locking assembly is installed.

It doesn't not get inserted like this. You take the threaded shoe off and install it through the back of the casting.
Here you can see the angles of the shoes and how they should face in order to lock the quill.
